Leadership Application
Discovery Church is led by a skilled team of leaders who are responsible for carrying out and leading the work of the church. Leaders include anyone who is responsible for leading a life group, a ministry or anyone working in our children and youth departments. Life Group Supplemental Application
Are you interested in starting a life group? If so, please take the time to fill out this short application that includes all of the details and plans you may have for your new life group. Please note that anyone who wants to lead a life group must have completed our leadership application. If you have not done so, you may attach the leadership application to this one.

Event & Facilities Reservation Form
This form is used by our members and leaders only. Please complete this if you plan to use any of the DC facilities for a special event or life group. Even if an event is not on the Discovery Church campus but you plan on it being hosted as a Discovery Church event and market it with our name and logo, we require that you fill out this form so that our pastoral staff can approve it and then put it on our public calendar.

Creative Arts Request Form
Do you want a cool flyer advertising your life group? Maybe you need information changed on our website. This is the form for you. All media to the membership of Discovery Church must have this form attached. Any print or email with the Discovery Church name and logo must be approved/produced through this form.
